What to include on a medication list
For each medication, list the name, dose, how often it’s taken, and what it’s for. Add the prescriber and your pharmacy, and call out drug allergies at the top. Include over-the-counter meds and supplements too — they interact. Bring it to every appointment and update it whenever something changes.
